I am constantly asking clients about their existing training needs.  I find that most of my client questions break down to a list of 15 that can usually provide me with a clear picture of what  is going on within a company.  This is a GENERAL survey that includes these questions  I would generally ask.  I am asking everyone visiting to take it.  It will only capture your responses.  There is no effort to capture your email or other information about who is completing it.  After 30 days, I will share the results.
